On November 7, 2025, at 1:35 PM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced at EES Vista Innovation Early Education Center (CCC) and was greeted by Director Desiree Buchanan. The purpose of the visit was to deliver the investigative findings regarding the allegations listed above. As part of the investigation, LPA conducted an initial visit on October 22, 2025, where census was taken, confidential interviews were conducted (S1-S5), and relevant documentation was obtained.

Community Care Licensing (CCL) received a complaint on October 21, 2025, alleging that staff yelled and hit a day care child. The complaint claimed that on October 15, 2025, child 1 (C1) was yelled at and struck on the thigh by staff 6 (S6). Four of four staff present on the day in question, deny witnessing or hearing C1 being yelled at or struck on the thigh. The name disclosed in the complaint, is not employed by the CCC or present on October 15, 2025. No additional witnesses or evidence were able to corroborate the allegation(s).

Due to conflicting statements and lack of corroborating evidence from witnesses, LPA was unable to substantiate the allegations regarding staff yell at daycare children, and staff hit daycare children. While these incidents may have occurred, they are not supported by sufficient evidence and are therefore deemed unsubstantiated.

An exit interview was conducted, and a copy of the report, appeal rights, and a Notice of Site Visit were provided to Director Desiree Buchanan. The Notice of Site Visit must remain posted in a prominent location visible to families and caregivers for 30 consecutive days.
