A case management visit is being conducted by LIcensing Program Analysts (LPAs) James Wilkerson & Susan Brewer in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 10/21/22. It indicates that Child #1 has been acting aggressively at the facility and that a parent/facility conference meeting was scheduled. The parent failed to meet with the facility and was a "no show". The parent has not been back at the facility for about a month and C1 has not been back since then as well. Based on information gathered, the facility acted appropriately and no violations have been identified.

An exit interview was conducted and a copy of this report was provided to facility staff.
