32 | Based on interviews conducted and records reviewed, when it comes to allegation that staff did not ensure children's diapering needs were met while in care, it was stated that Child 1's (C1) parent notified staff at drop off that the child had a dirty diaper but that Staff 1 (S1) stated they were not the teacher assigned to the child, therefore they would not be changing the diaper. S1 was interviewed and denied being told C1 had a dirty diaper and denied stating they would not change the diaper. It was also stated that during C1s drop off, S1 was changing another diaper and C1 ended up getting changed when the next teacher came in, approximately 12 minutes later.
When it comes to allegation that staff did not report incidents regarding children in care, it was alleged that C1 was bit while in care, but the parent was not notified of the injury. Based on a review of C1s file the parent was provided and signed for an ouch report. The ouch report was signed by the Director on 11/21/24, however neither the parent nor teacher provided date of when the report was signed for.
Lastly, based on interviews conducted, when it comes to allegation that staff did not prevent day care child(ren) from biting another child, interviews with staff revealed that teachers will try and redirect children if they know they are going to bite another child, but they don’t always know when it’s going to happen. Other staff interviews revealed that biting hadn’t really been a problem and that they were only aware of one biting incident.
Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED.
An exit interview was conducted, and this report was reviewed with the Director, Shoneil Wilson, and a copy was provided. Appeal rights were discussed and provided during the exit interview.
A Notice of Site visit was given, and Director understands that it must remain posted for 30 days. |
