13 | On 2/11/2026 at 2:45pm, Licensing Program Analyst (LPA) Adriana Macias conducted an initial unannounced complaint inspection for the purpose of investigating the above allegation. LPA met with staff present Olivia Fleet and later Site Director Francis Torrijos and Area Manager Kathryn Czerwinski arrived and LPA discussed the reason for the visit. During inspection LPA took census of the facility, obtained a LIC500 Personnel Report and Children’s Roster and conducted staff interviews. The following census was observed: 32 children and 3 staff. All staff had fingerprint clearances or exemptions.
It was alleged that a staff member (S1) employed by the facility exposed a child’s personal information through a social media posted video. Based on LPA’s interviews and video observed, it was discovered that S1 did in fact disclose and made a child’s personal information available online.
The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 1) the Type B deficiency is being cited on the attached LIC 9099D. LPA delivered findings to Director Francis Torrijos. Exit interview was conducted and report was reviewed with Director Francis Torrijos. A notice of site visit was given and must remain posted for 30 days.
