On 08/04/2022 at 3:00 PM, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced Case Management visit for the purpose of following up on a self-reported Unusual Incident. LPA met with Director, Cynthia Quintana and informed her of the purpose of the inspection. There were 42 children present with 14 staff at the time of the inspection.

On 07/21/2022 Director called Licensing Duty line to report an Unusual Incident that occurred on 07/15/2022 between 4:50 -5:00 PM. Around 4:45 PM, Staff 1 (S1) and Staff 2 (S2) were supervising 8 children ages 2-4 on the outdoor playground. While the staff were lining children up to transition from the playground to the classroom an individual approached outside the playground fence with toys and began a conversation with S2 about donating the toys to the facility. Also at this time a parent came to pick up their child and began a conversation with S1. During this time Child 1 (C1) left the waiting line unnoticed and was left on the enclosed playground alone as the Staff and the rest of the children went inside the classroom. Another parent arrived a few minutes later to pick up their child and noticed the child on the playground alone and brought it to the attention of the S1. C1 was brought back to the classroom and found to be calm and unharmed. Director stated she was not at the facility at the time of the incident and was notified about the incident by S1 at 5:30 PM. Director stated she mailed her written Unusual Incident report to Licensing on 07/18/2022 and attempted to make a verbal report to the Duty Line the same day but was only able to leave a voicemail message.

During this inspection LPA collected facility records, interviewed Director, S1, S2 and two daycare children. Due to insufficient information at this time, further investigation into this incident is needed.

Exit interview conducted and copy of report left with Director. Notice of Site Visit must be posted for 30 days.
