25 | On 08/17/2022 at 3:10 PM, Licensing Program Analyst (LPA) Dana Stevens, conducted an unannounced Case Management visit for the purpose of follow-up on an Unusual Incident Investigation. LPA met with Cynthia Quintana and informed her of the purpose of the visit. There were 13 children and 3 staff present at the time of this inspection.
On 07/15/2022 around 4:45 PM, while Staff 1 (S1) and Staff 2 (S2) were transitioning 8 children from the outdoor playground to the Preschool 1 classroom. Child 1 (C1) left the waiting line unnoticed and was left unattended on the outdoor playground. C1 was discovered a few minutes later by a parent and was brought to the attention of S1. C1 was found to be calm and unharmed. Director stated she was not at the facility at the time of the incident and was notified about the incident by S1 at 5:30 PM that evening. Director stated she mailed her written Unusual Incident report to Licensing on 07/18/2022 and attempted to make a verbal report to the Duty Line on the same day but was only able to leave a voicemail message, actual verbal report was made to Duty Officer on 7/21/2022 and written report (LIC 624) was received by Department 07/22/2022.
During this investigation LPA collected facility records and documentation, interviewed Director, S1, S2, daycare children, and daycare parents. Based on the evidence obtained in interviews it is determined that an Absence of Supervision violation occurred, and an Immediate Civil Penalty is being issued.
California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D.
Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
An exit interview was conducted with the licensee. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. Notice of Site Visit must be posted for 30 days. |
