25 | On 02/08/2024 at 10:25 a.m., Licensing Program Analyst (LPA) Cindy Meier conducted an unannounced case management inspection at the facility. LPA met with Administrative Director, Brenda Casillas and toured the facility. There were one hundred thirty-two (132) children present and nineteen (19) staff during the inspection.
In reviewing sign in and out sheets obtained during a complaint investigation, it was determined that on several occasions, on 6/23/23 and the week of 6/26/23 through 6/30/23, facility staff members signed children in and/or out, using their printed name. It was also determined that staff are allowed to sign children in and/or out, using another staff member’s iPad device without switching to their name. This practice is contrary to the facility Plan of Operation and California Code of Regulation (CCR), Title 22, Section 101229.1(b), which requires the person who brings the child to, and removes the child from, the center to sign the child in/out.
Pursuant to CCR, Title 22, a Type B deficiency is cited for the above described violation. (refer to LIC 809-D).
Exit interview conducted and report was reviewed with Administrative Director, Brenda Casillas. A copy of this report, along with Appeal Rights (LIC9058), were provided. A Notice of Site Visit was given and must remain posted for 30 days. LPA observed that the Notice of Site Visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
