13 | On 4/26/24 at 12:30pm, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the allegation listed above. LPA met with Administrative Director Brenda Casillas to discuss the purpose of the inspection and toured the facility. LPA conducted staff interviews on this date. It was alleged that Day care child sustained injuries due to staff neglect. During the course of the investigation, unannouced inspections were conducted, staff and parent interviews were conducted, and facility incident reports were obtained whic advised parents of incidents. LPA Castellon could not corroborate the allegations. Due to conflicting statements obtained during the course of the investigation, the above allegation is deemed to be UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged allegation occurred.
Exit interview was conducted, report reviewed, and Appeal Rights discussed with licensee. A Notice of Site Visit was given and must remain posted on, or immediately next to, interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
