25 | On August 9, 2022, at 2:37 PM, Licensing Program Analyst (LPA), Marie Hernandez, conducted an unannounced Case Management Inspection due to two incidents of 07/26/2022 and 06/09/2022 with child #1 and child #2. On 07/26/2022 and 06/09/2022, the Department received the incident reports from the facility. LPA met with the facility director, Tina Prowant. LPA toured the facility and play yard for health and safety. LPA conducted several confidential interviews with the staff, and the Director. The facility has a surveillance system indoors and outdoors. LPA reviewed the video footage of both incidents. The incident of 06/09/2022, the video footage shows child #1 playing outdoors in the play yard, pushing a Tonka truck, and losing his balance. The staff was nearby child #1 and tended to his needs. The child’s parent was contacted and took child to the doctors. Child #1 sustained an injury to his forehead which required medical services. As per the interviews and video footage, the incident of 06/09/2022 with child #1 was an accident.
LPA reviewed the video footage of the incident of 07/26/2022, where another child pushes child #2 to the floor in the classroom. Two staff were present including child #2's parent. The staff immediately tended to child #2's needs and redirected the child that did the pushing. As per the interviews, and video footage, this incident was an accident. The children continued to attend the facility after the incidents but to date, are no longer attending due to aging out.
The Director and staff denied that child #2 was being bullied, hit, sustained a bloody nose, and that someone hit child’s private area. Due to conflicting information obtained, the incidents could not be determined whether they occurred or not.
An exit interview was conducted with the Facility Director, Tina Prowant and the notice of site visit was provided to the Director. |
