On 6/12/2024, at 11:00am., Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced case management inspection to follow up on a self - reported incident. LPA met with Assistant Director, Krizia Beltran and Facility Representative, Jennifer Quintero. LPA discussed the purpose of the inspection and proceeded to tour the facility. There were 57 children present with sever (7) staff.

On 6/4/2024, Assistant Director, Krizia Beltran self- reported an incident involving Child 1 (C1). Per Assistant Director, the alleged incident occurred on 6/3/2024 at 10:35am. C1 sustained an injury while playing on the playground. Interviews were conducted with the facility representative, assistant directors and staff members. C1 was unavailable for interview. LPA inspected the playground, reviewed staff files, children records and video footage. LPA obtained copies of facility roster and other pertinent documentation

No deficiencies cited during today's inspection. Exit interview was conducted with Assistant Director, Krizia Beltran and a copy of this report, Appeal Rights and Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
