On 10/25/22 at 9:20 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct an announced pre-licensing (change of ownership) inspection. Upon arrival LPA, met with applicant, Nicole Hall and proceeded to tour the facility. The facility is requested a capacity of 34 preschool children; serving ages 2 years old through kindergarten in the palm classroom. Facility plans to operate Monday through Friday from 7:00 a.m. to 6:00 p.m.

The palm classroom was inspected and measured during this inspection and measured a total of 1,201.77 square feet, which is sufficient to accommodate 34 children at one time. The classroom is equipped with age appropriate furniture and play equipment; including napping equipment (mats), tables, chairs, cubbies, bookshelves, and other materials for the children use. The classroom is orderly, free of hazards and has adequate heating, lighting and ventilation. The children will utilize a restroom containing three sinks and three toilets. There are two sinks available for the children in the classroom. There is a total of three toilets and five sinks available for children's use in the facility. The number of toilets and sinks is sufficient to accommodate the requested capacity of 34 children. The restroom and hand washing areas are in a safe, sanitary and operating condition, per observation. The facility will provide AM and PM snacks to the children. Breakfast and lunch will be brought from home. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of filtered water bottles provided by the facility. The staff restroom located in the classroom will be designated for ill children. The director’s office will be designated as an isolation area for ill children. There is a functioning carbon monoxide detector and a first aid kit with required supplies located in the classroom, per observation. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible to the children in care. The facility will be utilizing manual sign in and sign out. Required documents were observed to be posted.

The outdoor preschool play areas were inspected and measured a total of 4,186.59 square feet which is sufficient to accommodate 55 children. The outdoor play area is fully fenced and includes outdoor play equipment. Play structure located in the back play area is suitable for age groups three through 10 years old. There is sufficient rubber matting material for cushioning to absorb falls. LPA observed adequate shade in the outdoor play areas by the use of canopies. LPA observed the outdoor activity space surfaces to be free of hazards. There is no evidence of rodent or insect activity, per observation. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children during outdoor play.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility plans to provide Incidental Medical Services – IMS. IMS plan on file and approved. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA informed applicant initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following with the applicant: PIN’s 21-04-CCP, 22-06-CCP and 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276).

The following is pending, prior to the licensure of the facility:
· Making the play structure age appropriate in the outdoor play area.

A fire clearance for 34 Preschool children; ages 2 years old through kindergarten in the Palm classroom was granted on October 12, 2022.

An exit interview was conducted with applicant, Nicole Hall. The following reports were discussed and provided: LIC809 pages and Appeal Rights (LIC9058). Once pending document is received and reviewed and after final file review a preschool license will be issued for 34 children; ages 2 through kindergarten in the palm classroom. Once license is issued the license will be mailed to the applicant and once received the license shall be posted in the facility.
