32 | Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. LPA and facility representative discussed water testing in child care centers and LPA provided an Effects of Lead Exposure flyer.

A Fire clearance for 60 children (age 2 – 5 years) was granted 7/5/22.
Prior to licensure the following is needed:
1) Waiver approval to share outdoor activity space
2) Waiver approval to share restrooms
3) Covers placed on all wall electrical sockets
4) Director exception approval
Exit interview was conducted with Facility Representative, Danielle Cavanaugh. Once the listed requirements have been met and proof submitted to CCL, a license may be granted after a final file review. |
