25 | On December 7, 2023., Licensing Program Analysts (LPAs) Van and Leong met with the Director, Sumer Al-Sardi, for an unannounced inspection of the Plan of Correction (POC). The purpose of the inspection was explained, and the Licensee granted LPAs entry to the facility. During the visit, two children were in care with a teacher and two co-op parents.
During the annual inspection on November 2, 2023, the Center was found to have multiple deficiencies. These included the failure to conduct fire and earthquake drills every six months, staff not completing the mandated reporter training, and water lead testing not being conducted before January 1, 2023, as required by the Directive.
During today's inspection, LPAs found that the log for fire and earthquake drills was appropriately documented and maintained, and water lead testing was done on November 16, 2023. All mandated reporter training for all staff has been completed, except for a substitute teacher who is not present every day. It was noted that all deficiencies cited during the November 2, 2023, inspection have been cleared today.
An exit interview was conducted with the Director, and a consultation was provided. In today's inspection, no deficiencies were observed. The Director, Sumer Al-Sardi, read and signed the report. Today's report and notice of the site were provided to the Director. LPAs informed the Director that a site visit notification must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
