25 | On 3/5/2025 at 3:45PM. Licensing Program Analyst, (LPA) Luis Gomez met with Director, Kris Taylor. The purpose of today's inspection was explained and was for unannounced, case management inspection, in response to an unusual incident report. Present was the Director and 5 staff supervising 15 children. Children present had been properly signed in. Facility was inspected for health and safety hazards.
On 1/21/2025., facility submitted an unusual incident reported to the Department. Per incident report, child in care, C1, sustaining an injury from child (C2) throwing an object and hitting them below the eye. Per director, staff immediately provided first aid to child’s injury and guardians of involved parties were notified.
During inspection LPA conducted interviews, observation, and record review.
Per director, facility has since created a behavior plan to better identify issues as they arise, and assist child (C2) before an incident occur. Facility staff are also being provided training to ensure preventative measures are implemented in the classroom to prevent injuries.
LPA reminded director, once facility has identified child as requiring elevated staff supervision, the facilities must provided a level of care that meets the child's classroom needs. Advisory Note: Technical Violation (LIC9102TV) was issued during inspection.

This report must be made available in facility for public review. Notice of site visit was given and must remain posted for 30 days.
