13 | On 6/27/2025, at approximately 9:00AM, Licensing Program Analysts (LPAs) Tran and Garcia conducted an unannounced, subsequent complaint investigation visit at the facility. LPAs met with facility representative, Selena Angeles, and explained the purpose of the visit. During the inspection, Director, Erica Nave arrived at the facility.
Facility is a combination infant and preschool center. Under the infant license, present during today’s visit included 3 children and 5 teaching staff, including the Director. Under the preschool license, present during today’s visit included 3 children and 3 teaching staff, including the Director. All staff present have fingerprint clearance on file.
Throughout the investigation, LPAs toured the facility, conducted classroom observations both indoors and outdoors, obtained and reviewed facility documents. LPAs also interviewed a random selection of staff, parents, and children.
Although the above allegation may have happened or is valid, based on LPA’s interviews, site observations, and record review which were conducted, there is not a preponderance of evidence to prove the alleged violation, teacher left kids alone, did or did not occur, therefore the allegation is UNSUBSTANTIATED.
No deficiencies were issued today during LPA’s visit. Notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with Director, Erica Nave. |
