Licensing Program Analyst, LPA Yee conducted a follow-up inspection today. The purpose of the inspection was explained. Present at the facility are site director/owner, Jie Min Zhang, 5 staff members, and 37 children (8 toddlers).

This is a follow-up inspection from a previous inspection which was conducted on 04/26/2023 to ensure the Type B violation was corrected.

Today LPA reviewed staff records. An advisory notice was provided.

The decision of an appeal correspondence letter was hand-delivered today.

The facility is licensed for 58 children. Under HSC1597.16. "A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test".

A civil penalty of $1200.00 was issued today. See next page for Type B citation.

Appeal Rights was provided to the site director.

Notice of Site Visit is to remain posted for 30 days.
