On November 02, 2023 at 1:50pm, Licensing Program Manager (LPM), Garfield Leung and Licensing Program Analyst (LPA), Winnie Ly met with Program Director, Kimberly Quinonez and Licensee Kristina Gonzalez for an Informal Office Meeting.

Discussed during the meeting was regarding a self reported Unusual Incident dated 09/21/2023 in which a 2 year and 9 months child exited the classroom through the emergency door exit, walk down the hallway and out to the front entrance of the building. The Program Director found the child standing by the front entrance.

Per Director, a meeting was held with the family to ensure the safety of their child under their care and supervision. As the facility’s plan of correction, staff have been reminded to make sure to always maintain 100% supervision of children. Since the incident, facility have done the following to prevent incident from happening again:

1. Facility have installed door bells for all emergency doors. The doorbells would sound if
someone open any of the door.
2. Facility has more staff per classroom.
3. Facility put stop signs on doors to help children understand they should not open the
door.
4. Facility staff doing head count of the children every half an hour.
5. Facility staff educate children not to open any door or leave the facility by themselves.

Received from Facility Representatives today are Staff Meeting Minutes as a proof staff were aware of the incident and how to prevent future incident, Pictures of door chimes and Letters to inform incoming parents and current parents regarding the incident.

Also discussed during this meeting were the citation and civil penalty issued for this incident. LPM also informed facility representatives that facility will be placed on required visit list.
