25 | On February 07, 2025 at approximately 10:30am, Licensing Program Analysts (LPAs) Ly and Naves conducted an Unannounced Annual Visit and met with Site Director, Kimberly Quinonez during the visit. Purpose of the inspection was explained. There were 11 toddlers and 23 preschool age children 12 staff members including the Director present during today's visit. Teacher to child ratios were met on this day. The facility has fully charged fire extinguishers that meet the minimum requirements, smoke detectors, and a carbon monoxide detector. The center uses an electronic system for parents to sign their children in and out of the program.
LPAs and Site Director inspected the center for Health and Safety Hazards. There are no pools, spas or other bodies of water on the premises. Per the Site Director, there are no firearms or weapons in the center. Adequate first aid supplies are available. All cleaning supplies and other potentially harmful items are stored inaccessible to the children. Medications are stored in inaccessible to children. Furniture and equipment are in good condition, free from loose, sharp or pointed parts. Children's restrooms are in working order with adequate supplies. There are separate restrooms on site for adults. The facility provides morning and afternoon snack. Children carry their own lunches to the program. All solid waste storage containers have tight fitting lids. Drinking water is provided children's individual water bottles.
Facility has waiver granted by the department to take children to the Golden Gate Park as part of their requirement for outdoor play. Last Emergency Drill was conducted on 10/17/2024 and properly logged.
During today's visit, a random sample of children's files were reviewed. Children's files contain emergency information, required licensing forms and immunization record.
LPAs also reviewed staff files. Based on file review, Four (4) staff files did not have Physician Health Screening form (LIC 503) and clearance for Tuberculosis. Staff did not have form on file poses a potential health and safety risk to children in care. Type B deficiency has been cited on this day in accordance with the California Code of Regulations, Title 22, see LIC 809D.
Based on staff file review, staff who declined to have flu vaccine did not have written statement on file. A Technical Violation has been issued on this day. Director has been advised to have staff file a written statement for declining flu vaccine.
