On March 7, 2025, at 2:00 PM, an informal office meeting was conducted at the San Bruno Regional Office. Licensing Program Manager (LPM) Leung and Licensing Program Analysts (LPA) Van met with the Director/Owner of the center, Wen Qi. The purpose of the meeting was to discuss the history of non-compliance, conduct inimical, and license capacity and limitations.

During the meeting, the following was reviewed and discussed:
  • During the annual inspection conducted on January 17, 2024, it was observed that the center enrolled two infants under two years old. In contrast, the license issued is designated for children up to two years of age to enter the first grade.
  • On January 17, 2024, three type B deficiencies were issued. Staff were missing mandated reporter training and immunization, and water lead testing was not completed before 1/1/2023.
  • On Jan 9, 2025, a type A deficiency was issued. An unknown person without criminal record clearance was present at the center while the children were in care. Two type B deficiencies were also issued; the Director was missing mandated reporter training, and cleaning products were accessible to children.
  • On February 7, 2025, a type A deficiency was issued. The Director intentionally altered the name on the mandated reporter training certificate.

During today's meeting, LPM reminded the Licensee of the following:
  • The director/owner must be aware of the department's regulations.

  • The director/owner must adhere to her license capacity and limitations.
  • Licensee/owner must maintain all records of enrolled children and update to date Children Roster
  • The director/owner must be truthful with Licensing personnel or representatives.
  • It is important to maintain precise records, and under no circumstances should these records be altered or falsified.
  • The Director/owner must ensure that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center.
  • The director/owner must review and inform center staff that any duly authorized officer, employee, or agent of the department may, upon presentation of proper identification, enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation of, this act or the regulations adopted by the department pursuant to the act.

Additional inspections may be required due to the center's history of deficiencies and non-compliance.

LPM explained that the Department's goal is to work alongside the center to ensure regulatory compliance. However, it is important for the center to follow the established regulations. Future noncompliance and further citations may lead to administrative action against the license.

An exit interview was conducted, and the report was reviewed with the Director/Owner, Wen Qi.
