On June 18, 2024 at approximately 9:00am, Licensing Program Analyst (LPA) Catrina Quimbo conducted a scheduled, pre-licensing visit. LPA met with applicant/licensee, Adriana Razo, and designated director for facility, Maria Murillo and explained the purpose of the visit. No children were present during LPA's visit.

This is a relocation application to the address above. The Crayon Box Preschool is currently licensed with facility 384002662. Relocation application was received January 26, 2024. The applicant is requesting for a preschool program capacity of 39 children (2 years to entry into first grade). Applicant plans to operate at facility July 8, 2024.

The facility will operate Monday through Friday 8:00am to 5:00pm. Applicant plans for children to be signed in and out of facility with an electronic signature. LPA reminded applicant and director sign in/out records must be made available for department's review. LPA advised applicant and director to maintain paper sign in/out records as a backup.

The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity. Facility is a multi-level facility that includes an outdoor area, large children's classroom, and teacher's area (second floor). Second floor will remain off limits to children. LPA observed a child safety gate to stairs that lead to second floor.

Indoors: There will be one large classroom for children's use. Classroom was measured for 1,223.32 sq. ft. allowing for a total capacity of 34 children. LPA observed classroom to be well ventilated with appropriate temperature. Classroom is equipped with furniture, toys and materials that were age appropriate and in good condition. Flooring in classroom was observed to be clean. LPA observed carpet in classroom to be free of stains. There is sufficient storage space for children's personal belongings located inside the classroom.

LPA did not observe any accessible poisons or cleaning solutions in classroom. Facility is equipped with a fire alarm system, fire sprinklers, multiple fire extinguishers and fully stocked first aid kit. Per applicant, smoke alarms in facility are dual smoke and carbon monoxide detectors.

Medication with children's individual names and required documentation will be stored in teacher's area. There is a refrigerator in teacher's area for medication that may require refrigeration.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Program plans to provide napping children with floor mats that are color coded. Children's families are to provide blankets and sheets. Per director, blankets will be sent home weekly or as needed for laundering. Floor mats will be labeled with children's individual names.

Applicant plans for isolation area to be in classroom, separate from other children in care. Program's discipline policy was discussed. Program plans to redirect children and maintain parent communication.

Classroom includes a children's restroom that includes 3 toilets and 1 urinal. There are 3 accessible sinks in the classroom and multiple drinking fountains. Total allowable capacity for toilets is 45 children. Total allowable capacity for sinks is 45 children. LPA observed toilet and sinks to be in working condition. Sinks accessible to children do not provide warm water. LPA observed garbage bins with tight fitting lids to be located inside children's restroom and classroom. There is a separate staff restroom located on site.

Program plans to provide a food service that includes morning breakfast and afternoon snack only. Children's families will provide lunch for their children. There will be food available on site if a child does not have lunch. Breakfast and snack will be stored and prepared in teacher's area on second floor. LPA reminded applicant and director children who bring their individual lunches to facility must be labeled with their individual names.

Outdoor: Children have access to outdoor area that is fully enclosed. Outdoor area was measured for 619.93 sq. ft. allowing for a capacity of 8 children. LPA discussed outdoor rotation requirement with applicant and director. Applicant plans to utilize toys and equipment from currently licensed facility to relocation facility. LPA observed outdoor area to be free of loose debris. Per applicant, outdoor toys and materials will include tricycles with helmets, easels and bouncy balls. There is sufficient shade in the outdoor area from neighboring building. LPA did not observe any pools, spas or bodies of water on site. LPA reminded director and applicant drinking water must be made available to children, indoors and outdoors.

The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Applicant was reminded a facility licensed after July 1, 2022, must have their water fixtures tested for lead 180 days after licensure and every 5 years thereafter. LPA advised applicant to review PIN 21-21-CCP.

LPA reviewed with applicant and director the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. LPA reminded applicant to conduct and document emergency disaster drills at least once every six months.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education
website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/ subscribe and select the Child Care option to receive email communication.

Based on square footage, maximum number program can be licensed for is 34 preschool children.

Prior to licensure, following must be completed:
-Fire clearance with updated capacity.
-Signed control of property submitted.
-Updated admission agreement and personnel report.
-Submission of outdoor rotational waiver.
-8 hour preventive health and safety certification for director.
-Sound alarm system on all exit doors.
-Covering of metal grate in outdoor area.
-Covering of holes in wall in outdoor area.
-Installation of carbon monoxide detector in classroom.
