Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi met with Director Sarina Escalante for the purpose of an announced pre-licensing inspection at 1240 Valencia Street San Francisco CA 94110 on 1/17/2025 at approximately 2:00pm. Applicant Barbara Walden was also present. Applicant is requesting 37 preschool children ages 3-5, 22 toddlers ages 18 months-36 months, and 16 infants ages 3-18 months. Facility will operate Monday through Friday from 7:00am to 6:00pm and drop-in hours are from 7:00pm-10:00pm.

Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. LPA provided and discussed LIC311A and the forms that must be in each child's and each staff member's file. Applicant stated the Needs and Services plans will be kept in the classrooms. Parents will provide formula, breast milk, and baby food for their children.

INDOOR ACTIVITY SPACE:

There is one infant classroom (#1), three toddler classrooms (#1,#2,#3) and two preschool classrooms (#1,#2) located in the facility. LPA measured each classroom. Infant classroom #1 contains a total 678.487 square feet and will accommodate the applicants request for 16 infant children. Toddler classrooms #1,2,3 contain a total of 1,060.205 square feet and will accommodate the applicants request for 22 toddler children. Preschool classrooms #1 and 2 contain a total of 1,762.907 square feet and will accommodate the applicants request for 37 preschool children. LPA observed a sufficient number of tables, chairs, and cubbies for 16 infants. LPA observed six cribs available for infants with padding that is at least 1 inch think with raised sides. LPA observed sufficient napping cots and mats in each classroom. LPA observed a variety of age-appropriate toys and equipment in each classroom. Applicant acknowledges there will be a maximum of six infants under 12 months old, due to available cribs in the infant classroom.

There is a first aid kit in each classroom and medications will be stored accordingly to each individual child. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children in each classroom in locked storage cabinets. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided by drinking fountains. LPA observed a functional carbon monoxide detector in each classroom. Applicant stated they will use a digital sign in/out system and paper sign-in/sign-out system as a back up. The isolation area for sick children will be located in the main office and the staff isolation area will be in the counseling room. The food preparation area is located next to the main office room and is inaccessible to children. Staff was reminded to keep all food preparation and storage areas clean and free of litter and rubbish to ensure the health and safety of children, visitors, and employees. There is a dedicated land line for the facility as well.

There is a total of 7 toilets, 21 sinks, 3 changing tables, and 6 cribs in the facility available for children and 2 staff restrooms in the facility. LPA observed a changing table within arm's reach of a sink with a changing pad that is at least one inch thick with raised sides that are at least three inches tall in the infant and toddler classrooms. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

OUTDOOR ACTIVITY SPACE
There are two outdoor play areas located at the facility, one for infant/toddlers and one for preschool children. The infant/toddler outdoor play area contains a total of 667.83 square feet and will not accommodate the applicants for 22 toddlers and 16 infants. The preschool outdoor play area contains a total of 1,641.14 square feet and will not accommodate the applicants request for 37 preschool children. LPA observed a sufficient amount of toys, play equipment, and materials for both infant/toddlers and preschool children. LPA observed sufficient resilient padding located in both the designated outdoor play areas. The outdoor play area is fenced by a concrete wall and metal fencing that is at least four feet tall. There is an emergency exit located in the outdoor play area which is locked from the outside and can only be opened from the inside by staff. Staff was reminded that the gate shall remain closed at all times during hours of operation to ensure the health and safety of children in care. There is sufficient shade for the outdoor play area provided by surrounding buildings, trees, and a shade sail. There are no bodies of water located on the premises.

LPAs discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with director any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, /chapter 676, Statues of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-year after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP)
