The building was under 2 addresses. 126 Lisbon Street, San Francisco, CA 94112, located on the ground floor, while 128 Lisbon Street is located on the upper floor of the building.
The Licensee owns this unit on the ground floor, consisting of entrance hallway, two bedrooms, a full bathroom, a living room, a laundry room, kitchen, basement, stairs to backyard and upper unit (128 Lisbon Street), emergency exit, and backyard on the ground floor. The home residents are Licensee and a Licensee’s minor child.
The childcare area will be provided in the living room, bedroom # 1, entrance hallway, kitchen, and bathroom on the ground floor.
The off-limit areas are bedroom #2, laundry room, stairs to backyard and upper unit (128 Lisbon Street), emergency exit, basement, and backyard on the ground floor.
The days and hours of operation will be Monday - Friday, 8:00 am to 5:00 pm, and care for children between 2 months old to 5 years old. Per the licensee, the licensee plans to utilize the bedroom # 1 as an isolation for sick/ill children.
LPA and the Licensee inspected the entire home for Health and Safety Hazards. The house is clean and orderly and has sufficient lighting and ventilation. LPA observed a playpen and four cots in the bedroom #1. LPA reminded the licensee to keep playpens and/or cribs free of toys, pillows, and hanging objects. Mattresses must be designed for playpens and/or cribs, and bed sheets should fit tightly. There are a variety of age-appropriate toys for the children. All furniture and playthings were observed to be in good repair.
