On 01/21/26, Licensing Program Analyst (LPA) Kesha Lewis made an unannounced visit to this facility to open an investigation for the above allegations. LPA identified herself upon arrival, stated the purpose of the visit. LPA met with Angela Riungu.

Based on documents received and interviews conducted R1 has not been in any physical altercations while in the facility and staff do not keep R2's Cigarettes. Therefore, the allegation Staff did not prevent residents from fighting is UNSUBSTANTIATED. A finding that the complaint allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted, and a copy of this report was provided.
