25 | On June 20th, 2024, at 9:30 AM, Licensing Program Analysts (LPAs), Janie Davis and Lauren Scott conducted an unannounced field visit to the facility for the purpose of a case management inspection to follow up on an incident reported to LPA Davis via email on June 7th, 2024. LPAs arrived at the facility and were met by Director, Lizbeth Arroyo. LPAs disclosed the purpose of the inspection and were granted entrance into the facility. 27 preschool aged children were present during today’s inspection. LPA determined, through accessing Guardian, that all required adults were background cleared and associated to the license.
LPA toured the facility, observed the care and supervision of children, reviewed records and conducted onsite interviews with Director and teachers. Interviews revealed that on Wednesday, June 6th, 2024, a child was left outside during transition without a hundred percent supervision.
One (1) Type A was assessed on a subsequent 809-D page. Director shall post LIC 809-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Licensees.
An exit interview was conducted, and the report was reviewed with Director. LPA provided Director with Licensee Appeal Rights. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
