Licensing Program Analysts (LPAs) David Nguyen and Deborah Khashe met with facility representative, Guadalupe Espinoza for the purpose of closing a complaint investigation. LPA Nguyen notified facility representative of the complaint. There were twelve (12) childcare children present and being supervised by three (3) childcare teachers. LPA Nguyen and Khashe toured the on-limit areas with facility representative during today’s inspection.

It was alleged that “Staff did not ensure sign-in and sign-out procedures were being followed” and “Staff did not provide authorized representative with child’s record.” During the investigation, LPA Nguyen requested facility roster and pertinent documents to assist with the investigation. LPA Nguyen inspected the facility and interviewed the childcare staff on site and reporting party via telephone. Based on interviews with childcare site supervisor, childcare teacher, childcare assistant, and reporting party, LPA obtained corroborating information from childcare site supervisor, childcare teacher, childcare assistant, and reporting party. Multiple individuals stated that there were multiple occasions where childcare staff members were signing in and/or signing out the childcare children due to the childcare parents and/or authorized representatives were not signing in and/or signing out the children at the childcare center during drop-offs and/or pick-ups.

In addition, based on interviews with childcare staff and the reporting party and records obtained during the course of investigation, LPA obtained corroborating information from the childcare staff as well as the reporting party, and records revealed that the childcare staff did not fully provide the child’s authorized representative with the child’s record upon request.

Based on corroborating information from childcare staff and reporting party and record reviews, there is a preponderance of evidence to prove the alleged violations did occur at the facility. Therefore, the above allegations are found to be SUBSTANTIATED.

The following Title 22 Deficiency is being cited on the subsequent 9099-D page.



An exit interview was conducted, and a copy of this report was provided and reviewed with the facility representative, Guadalupe Espinoza. Appeal Rights and a Notice of Site Visit were provided. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Center staff will make sure that childcare children are properly signed in and out with full legal signatures per licensing regulation. Facility representative will submit to LPA the copies of sign-in and sign-out sheets for the month of May to show compliance with the licensing regulation.
