13 | On September 27, 2023, Licensing Program Analyst (LPA) Stacey Williams arrived at the facility for the purpose of delivering complaint findings. LPA met with the Director, Kelly Reinbolt. LPA observed 87 children supervised by 9 staff.
An investigation was conducted regarding the allegation listed above. Interviews were conducted with the Reporting Party and facility staff. LPA obtained pertinent information throughout the course of the investigation that assisted with the determination for the complaint finding. The Director reported being unaware of commingling of preschool and school age children occurring at any time of the day. During the investigation, video footage was reviewed of the facility. This video footage revealed staff leaving before the facility's closing hours as well as staff placing preschool and school age children in the lobby area with Administrative staff supervising the children until their parents arrived. A review of the facility file was conducted, and there are no waivers on file authorizing commingling at any time of the day.
Based on the information received the allegation is determined to be substantiated. Title 22 deficiencies will be cited on subsequent page, LIC 9099D.
Exit interview conducted at which time the report was reviewed with Facility Representative Kelly Reinbolt. A Notice of Site Visit was posted by LPA Williams and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. |
