25 | Licensing Program Analyst (LPA) David Nguyen met facility representative, Merary Picos, to conduct a proof-of-correction inspection to verify corrections of one (1) Type-A deficiency cited on July 29th, 2025. The purpose of the proof-of-correction inspection was explained. There were forty-two (42) childcare children present being supervised by six (6) childcare teachers during today’s proof-of-correction inspection.
On July 29th, 2025, the childcare facility was cited for one (1) Type-A deficiency for not maintaining proper child-teacher ratios. During the inspection on July 29th, 2025, LPA observed forty-nine (49) childcare children were left with three (3) childcare teachers during outdoor playtime. During today's inspection, LPA toured on-limits areas. LPA observed supervision and ratios were appropriate. The Type-A deficiency cited on July 29th, 2025, for not maintaining proper child-teacher ratios was cleared on July 31st, 2025. In addition, LPA Nguyen received the sign-in sheet of the teachers’ meeting and teachers’ meeting agenda, which was held on July 30th, 2025, from facility representative during today’s inspection.
In addition, on July 29th, 2025, the childcare facility was cited for one (1) Type-B deficiency for three staff members out of six staff members not having valid child care provider mandated reporter training certificates in files. During today’s inspection, LPA reviewed the teachers’ licensing files. LPA observed the teachers’ certificates of completion for child care provider mandated reporter of three (3) teachers. The Type-B deficiency cited on July 29th, 2025, for three staff members out of six staff members who did not have valid child care provider mandated reporter training certificates was cleared on July 31st, 2025.
