On May 21st, 2026, at 2:15 PM, Licensing Program Analysts (LPAs), David Nguyen and Deborah Khashe met Facility Representative/Director Tyanne Woodyard Smith for the purpose of initiating and closing a complaint investigation. The purpose of the inspection was explained. LPAs toured the facility. There were thirty-one (31) children present and being supervised by six (6) childcare staff.

It was alleged that "Daycare child was left outdoors without staff supervision." During the investigation, LPA Nguyen inspected the facility, observed the care and supervision of children, and interviewed the Director and the teachers. The facility self-reported the Unusual Incident Report (UIR) occurred on May 19th, 2026, where a childcare child was left alone unattended on the playground during the transition to the classroom, to California Department of Social Services (CDSS), Community Care Licensing Division (CCLD) on May 20th, 2026 via email.

Based on the information obtained during today’s inspection and records review, LPAs learned that Child # 1 was left alone unsupervised on the playground during the transition to the classroom.

Based on corroborating information from childcare staff and reporting party and record reviews, there is a preponderance of evidence to prove the alleged violation did occur at the facility. Therefore, the above allegation is found to be SUBSTANTIATED.

The following Title 22 Deficiency is being cited on the subsequent 9099-D page.



LPA David Nguyen informed facility representative/Director, Tyanne Woodyard Smith that this report dated 5/21/2026 documents one (1) type-A citation. Type-A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA David Nguyen informed the facility representative to provide a copy of this licensing report dated 5/21/2026 that documents any Type-A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement or Receipt of Licensing Report (LIC9224) or other written statement, must be in the child’s file for verification.

LIC9224 and Appeal Rights were provided. An exit interview was conducted, and the report was reviewed with Facility Representative/Director, Tyanne Woodyard Smith. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Facility Representative/Director, Tyanne Woodyard Smith acknowledges that for TYPE-A DEFICIENCIES ONLY upon receipt, Director shall post LIC 809-D.
