On February 20, 2026 at 3:22 PM, Licensing Program Analysts (LPAs) Bill Billones and Elizabeth George conducted an unannounced inspection of the Child Care Center (CCC) to initiate and deliver findings for the above allegations submitted to the Department. LPAs met with Director Angelica (Gallardo) Jepsen. At the time of the inspection, LPAs observed 37 children under the care and supervision of 8 staff.

The complaint alleged violations of: Record Keeping, specifically staff are not ensuring sign in/out sheets are being filled out properly; and Personal Rights, specifically staff had inappropriate interaction with daycare child. During today’s investigation, LPAs reviewed the facility’s Guardian roster and the facility’s sign-in/out sheets for the children. LPAs also conducted on-site staff interviews.

LPAs observed that the children’s sign-in/out sheets were incomplete, corroborating the allegation regarding Record Keeping. Additionally, staff interviews conducted by LPAs confirmed that an inappropriate interaction occurred at the center, supporting the allegation related to Personal Rights.

Based on LPA's observations and interviews, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED.

As a result, two Type B deficiencies are being cited today under California Code of Regulations, Title 22, Division 12 on the attached LIC9099Ds. Director was reminded that this report must be made available to the authorized representatives of children in care and maintained in the facility file for three years.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted, appeal rights were provided, and report was reviewed with the Director Angelica (Gallardo) Jepsen.
