25 | At 02:05pm on 03/24/2025, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the scheduled time to conduct a follow-up to the facilities pre-licensing inspection. LPA met with Licensee/Administrator Amelita Aquino, announced who he is and the reason for the visit.
Administrator and LPA conducted a physical tour of the facility. This facility has a centrally located full kitchen, two living rooms, dining area, 4 resident bedrooms (two of the resident bedrooms are double resident occupancy and two are single resident occupancy), and there is a staff bedroom. There are three full bathrooms. The primary bedroom has an on-suite bathroom, resident room #3 and the staff bedroom share a "Jack and Jill" style bathroom, there is a full public bathroom at the front of the facility for visitors and bedrooms #1 & 2. The laundry room door is now locked for resident safety. There are two outside areas for residents and visitors to enjoy. The front of the facility has a small area for visitation and the backyard has a covered patio for shade and is large enough for outside activities. LPA tested and observed the the smoke detectors to be working. There are two carbon monoxide detectors plugged into outlets in the hallway, also tested and functioning. LPA observed a medication closet that has a lock on the closet door in the hallway next to the laundry room. LPA observed a new and complete first aide kit in the medication closet. LPA noted that there is a working fire extinguisher in the kitchen area. LPA noted that the facility is clean and in good repair, hallways and doorways are free and clear of debris. LPA noted that screens on window and door slider in resident bedroom #4 have been repaired. Water temp tested at 118.9*F within the required 105-120*F. Facility heated to temperature of 72*F.
Administrator and LPA conducted a review of the pre-licensing care tools. On 03/11/2025 during original pre-licensing visit, Administrator and LPA's conducted a full review of the Comp III training tools.
This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license.
