25 | On 2/9/26, at 1:05 PM, Licensing Program Analyst (LPA) Matthew Sapien conducted an unannounced annual random inspection of the abovementioned Family Child Care Home (FCCH). LPA met with Martha "Kathy" Gutierrez, Licensee of the FCCH, and explained the nature and purpose of the inspection. The LPA, in the company of Licensee, toured the FCCH in its entirety (inside and outside). The home is a 4 bedroom and 4 bathroom two story home. The FCCH utilizes a dining room, entry room, one hallway bathroom, and backyard. The remainder of the home is excluded which includes the kitchen, living room, all four bedrooms, three bathrooms, garage, laundry room, and front yard. Importantly to note, the entirety of the second story is off limits and made inaccessible by a child safety gate at the base of the stairwell. At the time of the inspection, LPA observed 3 napping children present, 2 of whom were infants. At the time of the inspection, LPA also observed the Licensee's biological child and mother (cleared and associated).
LPA observed the FCCH to be clean and orderly. The bathroom, utilized for childcare, is clean and free of toxins. LPA observed bathroom cabinets to be free of any potentially hazardous items. LPA observed sharps and personal medications in elevated cabinets and shelves in the kitchen. No children currently require medication. Cleaning supplies are stored in elevated areas in the laundry room, an area that is off limits for children in care. LPA also observed a First Aid Kit in the residence with appropriate supplies.
LPA observed age-appropriate toys, furnishings, and equipment throughout the entry and dining room of the residence. In the living room, an area off limits to children in care, LPA observed a fireplace that was enclosed by a glass screen. Licensee informed LPA that the fireplace is not used during day care hours. LPA observed one larger dog present on site. Licensee informed LPA that the pet is current on their vaccinations and has no interactions with children in care. LPA observed relevant licensing forms and documents posted prominently to a wall at the entry way of the residence. LPA observed separate carbon monoxide and smoke detectors near the dining room of the residence. A smoke detector was tested at 1:45 PM and a carbon monoxide detector was tested at 1:40 PM and each were found to be operational.
LPA observed a regulation fire extinguisher in the FCCH which was last serviced on 5/20/25. LPA reminded the Licensee to either service or purchase a regulation fire extinguisher annually. LPA reviewed the FCCH's (CONT. 809-C, Page 2)
