25 | On 12/30/25, at 10:20 AM, Licensing Program Analyst (LPA) Matthew Sapien conducted an unannounced annual random inspection of the abovementioned Family Child Care Home (FCCH). LPA met with Heather Luis, Licensee of the FCCH, and explained the nature and purpose of the inspection. The LPA, in the company of Licensee, toured the FCCH in its entirety (inside and outside). The home is a 3 bedroom and 2 bathroom single story home. The areas that are accessible for day care children are as follows: living room, dining room, all three bedrooms, one hallway bathroom, and backyard. The remainder of the home is excluded from childcare services which includes the garage, master bathroom, and a section of the backyard. In the home, inaccessible areas and rooms are made secure by locks and gates. At the time of the inspection, LPA observed 1 child present, which of whom was an infant (Licensee's biological child). Besides the Licensee, the LPA also observed the Licensee's husband present at the time of the inspection (cleared and associated).
LPA observed the FCCH to be clean and orderly. The bathroom, utilized for childcare, is clean and free of toxins. LPA observed bathroom cabinets to be free of any potentially hazardous items. LPA observed cleaning compounds in elevated areas in the kitchen, hallway bathroom, and hallway cabinets. Personal medications are stored in an elevated cabinet in the kitchen. No children currently require medication. Sharps are also stored in the kitchen in an elevated cabinet.
LPA observed age-appropriate toys, furnishings, and equipment throughout the indoor spaces for the day care. LPA observed an electric fireplace in the living room, which is enclosed by glass and is not used during day care hours. LPA also observed a dog and a cat present on site, each of whom are vaccinated and have no interactions with children in care. LPA observed relevant licensing forms and documents posted prominently to a wall at the entry way of the residence. LPA observed a number of carbon monoxide and smoke detectors in the residence. A smoke detector and a carbon monoxide detector were separately tested at 10:54 AM and were found to be operational. (CONT. 809-C, Page 2)
