On 8/28/25 at 1:05 PM, Licensing Program Analysts (LPAs) Matthew Sapien and Seena Parsapour conducted an unannounced inspection to the abovementioned Child Care Center (CCC) in order to investigate the above allegation. LPAs met with Kelsey Sullivan, Director of the CCC, and explained the nature and purpose of the inspection. LPAs, in the company of Director, toured the CCC. At the time of the inspection, LPAs note 25 children present, along with 2 staff assistants (cleared and associated). Upon intital arrival, one of two staff assistants were taking their lunch break.

The investigation included interviewing the Director on 7/17/25, as well as interviewing a sampling of parents of children in care. Pertinent documents and observations were also made by LPAs. As noted, the complaint alleges the CCC violated proper staffing to child ratio requirements.

Interviews with the Director and parents of children in care did not corroborate this allegation. On the contrary, interviews with parents of children in care revealed that they have observed an adequate number of (CONT. 9099-C, Page 2)

Although the allegation may have been with merit, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore, the allegation listed above is deemed UNSUBSTANTIATED.

An exit interview was conducted with Facility Representative, Kelsey Sullivan.

Facility Representative was provided with Appeal Rights (LIC 9058) and a Notice of Site Visit (LIC 9213). Notice of Site Visit must be posted for 30 days or a civil penalty of $100 may apply.
