On October 23, 2024 at approximately 2:15pm, Licensing Program Analysts (LPA) Melissa Zaragoza conducted an unannounced, case management visit to Highlands Christian Schools. LPA met with director, Crystal Camacho, and associate director, Samaa Srouji, and explained the purpose of the visit. Present during LPA's visit included 12 teaching staff and 52 children present. Facility is operating within capacity limits and ratio during LPA's visit. Teaching staff present have fingerprint clearance on file. Childcare centers hours of operation are Monday through Friday, from 7:30am-5:30pm.

LPA conducted a visit in regard to two unusual incident's reports that were reported to the department. First unusual incident was reported on October 9, 2024. The incident involves Child C1 informing their parent of having a scratch. Per director, the childcare center reviewed footage from the cameras of the day of the incident and did not see C1 injury themselves. Per director, it was a normal day for C1. Per director, mother of C1 took C1 to the doctor on 10/7/24, and according to physician, there is nothing unusual about the injury. Per mother of C1, the injury was a small scratch, the size of a fingernail. Doctor did not prescribe any medication or further instructions for the injury.

Per director, the child has returned to the childcare center. C1 attends program on Mondays, Wednesday’s, and Fridays only. Per director after the incident, C1 was absent due to a stomach bug, and missed 10/9/24 and 10/11/24. Per director, C1 has not complaint about injury since, and is constantly taken to the restroom for bathroom breaks.

Second Unusual incident report was reported to the department on October 11, 2024. The incident involves Child C1 who was upset at a staff member and jumped on the staff members back. Per director, the staff involved went to the hospital the following day, 10/12/24. Staff injured is currently under workers comp and working with restrictions. Per director, staff is attending physical therapy. Staff has a follow up appointment with an occupational medical clinic on 10/29/24.

Per director, before the incident, C1 was going to switch classrooms to a different class. Per director, due to the incident, the transition of a change of classroom was made sooner. Per director, they had a meeting with C1s parent and constantly check in with them. Per director, mother of C1 and childcare program came up with a plan for C1 that can help C1 have a better experience at the facility.

Per director, a staff meeting took place on October 14, 2024. Staff members that attended meeting signed their signature in a form for proof of attendance Childcare center provided LPA with signature list of staff. Per director, the following were discussed in the meeting, Title 22 regulations, parents’ rights, children's rights, and supervision. Per director, teachers were also explained, if they feel overwhelmed to please contact the office for support or use Bright Wheel to contact other teachers.

A copy of this report, and the “Notice of Site Visit,” were given to the director.

A “Notice of Site Visit” must remain posted for 30 days.

An exit interview was conducted, and the report was reviewed with facility representatives, Crystal Camacho.
