On July 9, 2024 at approximately 8:30am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with administration assistant, Shirley Ngai, and center director, Cynthia Huang, and explained the purpose of the visit.

Upon LPA's arrival, children were arriving to facility. Hours of operation are Monday through Friday 7:30am to 6:00pm. Facility is a multi-level building that includes 2 stair cases. LPA observed stairs cases to include a child safety gate at the top and at the bottom of the stairs. Program is licensed to operate in classrooms #1 through #12, three play yards ("Jungle", "Forest", and "Yellow building playground") and Parish hall (food purposes).

During LPA's visit, preschool program is currently operating in classrooms #4 through #12: Ducklings (2 years olds in Room 4), Colts (3 year olds in Room 5), Dolphins (3-4 year olds in Room 6), Bunnies (3 year olds in Room 7), Owls (4 year olds in Room 8), Bears (4 year olds in Room 9), Penguins (3-4 year olds in Room 10), Alligators (3-4 year olds in Room 11), and Lemurs (TK age in Room 12). Preschool program is also utilizing all three play yards.

Present during LPA's visit included 139 preschool children and 21 preschool teaching staff (including director and administrative assistant). Staff present have fingerprint clearance on file. With administrative assistant, LPA inspected day care areas in use, indoors and outdoors, for health ad safety hazards.

LPA observed all classrooms to have appropriate temperature and ventilation. All classrooms were observed to be clean, equipped with appropriate toys, materials and furniture. Classrooms are sectioned into areas for activities, circle time, and quiet/rest areas. LPA observed flooring in classrooms to include carpets, mats and rugs that are clean and in good condition.

Each classroom includes a sink that LPA observed to be working. Per administrative assistant, sinks in classrooms are for hand washing only. Sinks accessible to children do not provide water that is warmer than 120 degrees Fahrenheit.

LPA observed all classrooms' toys to be in good condition. Materials are labeled and stored appropriately. Per administrative assistant, children's toys and materials are sanitized daily while entire classroom is deep cleaned multiple times a year.

LPA observed cleaning solutions, poisons and chemicals to be made in accessible in all classrooms, stored in high shelves and/or locked behind child safety locked cabinets. LPA observed classrooms to have a smoke/fire alarm system, fire sprinklers, multiple fully stocked first aid kits and a working phone. LPA observed electrical outlets to made inaccessible with child safety covers.

There is sufficient storage for children's belongings, located inside each classroom. LPA observed cubbies for children's use to be appropriately labeled with children's individual names. Classrooms were observed to have licensing documents properly posted and made available for review. Children are signed into program with electronic signatures on an electronic application. Per director, communication is maintained with families via electronic application as well.

Children sleep on cots. LPA observed cots to be stored appropriately. Children's sheets are stored in bags that are labeled with their individuals names. LPA observed children's sheets to be stored appropriately so that they are not touching each other. Per administrative, assistant, sheets are laundered weekly or as needed.

Children's restrooms are located on each level of facility. Children's restrooms are separated between "Boys" and "Girls". LPA observed restrooms to be in working condition, with appropriate sanitation materials. Children in diapers are changed in bathroom. LPA observed garbage bins with tight fitting lids for dirty diapers. Per administrative assistant, garbage bins are emptied daily by janitorial services.

Outdoor area for children's use includes the "jungle" (located outside room 8), "forest" (located outside preschool office), and "yellow building playground". Outdoor areas include play structures that have resilient padding underneath. LPA did not observe any bodies of water on site. Water is made available to children with children's individual water bottles, labeled with individual names, that can be refilled on site through filtered water station.

LPA reviewed facility records that included 10 random children's records and 10 random teaching staff present. LPA observed children's records to be complete with emergency information and immunizations. Teaching staff records were also observed to be complete with current Mandated Reporter teaching certifications, required immunizations and education transcripts. LPA observed all 10 teaching staff records to have a current CPR/First Aid certification that will expire 08/2024.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records. For IMS information see PIN 22-02-CCP. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the director, Cynthia Huang.
