On July 18, 2024, at approximately 8:30am, Licensing Program Analysts, (LPAs) Alvarado and Tapia-Mandujano conducted an inspection and met with Director, Kaman “Carmen” Lo. Present in the facility are Director Kaman Lo and Eight staff members supervising 36 children ages 6 months-3 years old. All adults present supervising children in the facility have criminal background clearances and are associated with facility.

During the Health and Safety inspection of the entire facility, LPA’s observed the facility to have some wear and tear throughout the classrooms and need additional maintenance to ensure the health and safety of children (Technical Violation was received).

At approximately 9:10am, LPA’s observed a larger crib in classroom 5 with an infant child inside of the crib with the latch lock faced forward and partially unlocked. At approximately 9:15am, in the back side of classroom 5/8 (sleeping room), LPAs observed a pink blanket in one of the infant cribs with no infants present. At approximately 9:35am, LPA’s observed an unqualified staff alone with a group of 3 infants leave the classroom. Per interview and observations, LPAs determined that infant sleep logs were not being maintained. Sleep logs that were provided to LPA’s were from March 2024 and prior. Due to this determination, there are potential health and safety risk for the children in care. Please see LIC 809D for more details and deficiency cited.

In classrooms 3/4, LPA’s observed 12 toddlers with 2 staff members. Approximately 3-5 minutes later, LPA’s observed one child being taken to the classroom next door (Classroom 2/1). Classroom 3/4 was operating over teacher child-ratio. Due to this determination, there was an immediate health and safety risk for the children in care. Please see LIC 809D for more details and deficiency cited.

During the inspection, LPAs provided Technical Assistance on Safe Sleep Regulations and Records to be maintained. LPAs also answered questions Director had around CCR regulations and provided resources.

***California Code of Regulations, (Title 22, Div. 12, Ch 3) were cited on this day.

One type “A” violation was issued today. Director is advised to provide a copy of the Evaluation Report and all Type “A” Deficiencies cited, to the parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be maintained in all Children's files. Notice of site Visit will be posted and should remain posted for 30 days, failure to post will result in an immediate civil penalty.

This report must be available in the facility for public review. An exit interview was conducted with Director, Kaman “Carmen” Lo. Appeal Rights were provided.
