NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On January 22, 2024, at approximately 9:40 AM, Licensing Program Analyst (LPA) Melissa Zaragoza conducted an unannounced, annual inspection. LPA met with the director, Corey Bianchi, and explained the purpose of the visit. Our lady of Angels is a preschool age program. Preschool program is located in the school grounds of Our Lady of Angels. Preschool program operates in two classrooms, the 4-year-old class, and the 3-year old class. Hours of operation are Monday through Friday 8:30am to 3:30pm and 8:30 am to 12:00pm. Present during LPA's visit was the director, 36 preschool age children and 5 teachers. Teachers are in the classroom with the children. Program is operating within capacity limits and ratio during LPA's visit. LPA inspected facility, indoors and outdoors, for health and safety hazards. Classroom includes a variety of toys and materials that were in good condition. LPA observed classroom to have age-appropriate furniture, equipment, and materials. LPA observed cleaning materials to be stored in high shelves, inaccessible to children in care. Per director, children do not sleep while they are in preschool. Per director, some children enrolled have a short schedule, when children arrive, they are in the class learning and working. After children are finished, some kids go outside for lunch, and some kids gets picked up. The classrooms are equipped with a fully charged fire extinguisher and first aid kit. LPA observed Carbon monoxide detector in the classrooms. Carbon monoxide detectors were tested during visit and were observed to be working. Smoke alarm was unable to get tested, due to alarm wired to the entire school buildings. Storage for children's belongings is in classrooms. Children's cubbies are labeled with children's individual picture and name on it. (Continue Report on Page 2...)

There is one bathroom in each classroom for the children to use. Bathroom for children was observed to be clean. Bathrooms in the class have 1 toilet and 1 sink. The sinks and toilet were in working condition. Bathroom was equipped with appropriate sanitation products. LPA observed bathroom to have trash can bins with tight fighting lids. LPA observed a box in the bathroom with extra clothes for each child, in case of any emergency. LPA observed no cleaning solutions, poisons, and other hazardous materials for to be stored in bathroom. LPA did not observe any accessible poisons or chemicals in classroom.

Program does not provide a food service. Per director, children bring snack and lunch from home. Per director, children store their lunch bags and water bottles in a lunch cart. Per director, children eat lunch inside the class.

Outdoor area is entirely enclosed and fenced. LPA observed outdoor play area to have natural shade. LPA observed play structure to have cushion flooring. Outdoor materials are stored outside, inside a shed. LPA observed a sandbox to be fully covered. Sandbox is covered with a tarp. LPA observed toys and material to be in good conditions. Per director, children take their water bottles outside, and refill them in the classroom. LPA observed filtered water in class.

Program uses a form for parents to sign in and out for their sign in/sign out system. Children were properly signed in during LPAs’ visit. LPA observed licensing posters and required documents to be properly posted inside the classroom. Program’s last disaster drills was conducted on January 7, 2025.

LPA reviewed 6 facility staff records. LPA reviewed 6 random children facility records which were complete and had all required documents. Children's records have emergency information on file. All staff on site have a current CPR/First Aid certification. LPA reminded director; mandated reporter training must be renewed every 2 years for all staff working with children. Staff records also include required immunizations that were made available for review.

Director was reminded all staff hired are required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Water quality was tested on 08/03/2022 and results complied.

LPA's discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were cited today.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the director, Corey Bianchi.
