13 | On 01/09/2023, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced inspection and met with Director, Jennifer Silva. Purpose of the inspection was explained, and it was to report the investigations finding for above allegations. Complaint was received by the Department on 11/14/22. Present in the facility are Director and seven staff caring for a total 11 infant children. LPA and Director inspected the facility for health and safety hazards.
During the course of the investigation, interviews were conducted with staff and parents, and pertinent documents were received. Based on evidence received, LPA cannot determine if a day care child sustained injuries while in care.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the allegations are closed as UNSUBSTANTIATED.
After today’s visit, an exit interview was conducted with Director, Jennifer Silva. A copy of this report was reviewed and provided to Director.
Upon receipt of this report, Facility shall post the Notice of Site Visit. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required, will result in an immediate $100 civil penalty. This report is public and can be reviewed.
