13 | On May 5th, 2025 at approximately 8:30am, Licensing Program Analysts (LPAs) Alvarado and Quimbo conducted an unannounced 10- day complaint inspection in response to the above allegation. LPAs met with assistant director, Jasmine Wood, and explained the purpose of the visit.
At approximately 9:00am, LPAs met with Executive Director, Ana Galdamez. LPAs explained the purpose of the visit. Present during inspection was Executive Director, 4 staff members supervising 13 children. All staff present have fingerprint clearance and are associated.
During today’s visit, LPAs conducted a health and safety inspection and obtained the following documentation: A copy of children's roster with parent contact information (LIC 9040) and Personnel Report (LIC 500). LPAs conducted a random selection of staff interviews and file review.
During today’s inspection it was observed that one infant teacher had stepped out with the children’s cart holding on to the stroller “buggy” that holds 6 infants. Infant teacher who stepped out does not have the qualifications to be left alone with the infants without the direct supervision of a qualified infant teacher. Ratio was also determined not to be maintained since staff had 6 infant children in the stroller.
During the random selection of staff interviews it was confirmed that rodents were observed and present during operating hours on multiple occasions. Facility had taken action in installing blockages for the bottom of the doors and sticky rat traps were observed.
Due to the information provided; therefore, the above allegations is found to be SUBSTANTIATED. Type B citation has been issued today, May 5th, 2025, in accordance with California Code of Regulations, Title 22, see the attached LIC 9099D staff infant ratio, regarding building and grounds.
A plan of correction of this report, appeal rights, and notice of site visit were discussed and provided to Executive Director, Ana Galdamez. Director was reminded that a Notice of Site visit (LIC 9213) must be posted for 30 consecutive days during the hours of operation after each site visit by a licensing representative Failure to maintain posting as required will result in a civil penalty of $100.00.
