On March 19, 2026, Licensing Program Analyst (LPA) Van conducted an unannounced annual inspection and met with the director, Marilyn Lazana. The purpose of the inspection was explained, and the director granted LPA access to the center. At the time of inspection, seven staff members, including the director, supervised 28 children. The facility complies with the current staff-to-child ratios.

All staff members present during the inspection had their criminal record clearances verified through the CDSS Guardian Background Check system and were confirmed to have an eligible clearance status.

The days and operations hours are Monday - Friday from 7:00 a.m. to 5:30 p.m.

The center uses the Kinder Connect electronic sign-in/sign-out system. Each parent or authorized representative is assigned a unique access code. A review of attendance records confirmed that all children were accurately accounted for and that parents consistently completed the required signing procedures.

LPA and the director conducted a tour of the center to assess potential health and safety risks. The Center has two spacious classrooms designed for different age groups: Room 1 for children ages 4–5 and Room 2 for children ages 2–3. All classrooms were observed to be clean, well-organized, and maintained at a comfortable temperature with adequate ventilation. Age-appropriate toys and educational materials were readily accessible, and all furniture was sturdy and in good condition. The Center is equipped with multiple combinations of carbon monoxide and smoke detectors, a centralized smoke alarm system, and multiple fully charged 2A:10BC fire extinguishers. The director confirmed that no firearms or weapons were present at the center.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed that each child has an individual cubby for storing personal belongings. Sleeping cots were properly stored when not in use. According to the director, the Center provides cots, and the family brings bed sheets and blankets. All napping equipment is returned to families at the end of each week for laundering. Emergency supplies are stored in locked areas to ensure they remain secure and inaccessible to children.

According to the director, there are currently no children enrolled who require medication this school year. The director explained that if a family provides medication for their child, all medications are stored in a centralized locked cabinet. Medication is then placed inside a locked metal box to ensure secure storage.

The director stated that the Center provides breakfast, lunch, and afternoon snacks. The kitchen area was observed to be clean, well-maintained, and securely gated to prevent children from entering. All food preparation surfaces were sanitary and free of debris. No signs of cross-contamination were observed, and all food items and utensils were stored appropriately. All solid-waste containers observed had tight-fitting lids. Drinking water is available to children both indoors and outdoors throughout the day.

There is a shared bathroom between the classrooms that contains three sinks and four toilets. The LPA observed the bathroom to be clean, well-maintained, and in proper working condition. The area was equipped with appropriate sanitation supplies, and all toileting fixtures were child-accessible and functioning safely. A separate staff restroom is also available on site.

Disinfectants, cleaning solutions, poisons, and other hazardous materials were stored out of children’s reach. All accessible cabinets and drawers in the classrooms were checked and found free of hazardous items.

The outdoor play area was observed to be clean, free of debris, and fully fenced. All equipment appeared to be in good repair. Play structures were age-appropriate and labeled for children ages 2 to 5 years. Soft cushioning material was installed beneath the play structure to reduce the risk of fall-related injuries. The director stated that the sand area is inspected daily before use. LPA reminded the director to ensure drinking water is available for children during outdoor activities.

All required posting documents were posted, including the Facility License, Notification of Parent Rights, Personal Rights, Car Seat Law, Emergency Disaster Plan, and Daily Activity Schedule.

According to the director, fire and earthquake drills are conducted monthly; the last drills were conducted on February 19, 2026.

LPA reviewed a sample of children’s and staff records during the inspection. Children’s files were complete and contained all required documentation, including immunization records, admission agreements, emergency contact information, and personal rights forms. Staff files confirmed that all employees and individuals subject to background checks had obtained the required criminal record and child abuse index clearances or exemptions. All staff members held current CPR certifications, maintained required immunization records, and were up to date on mandated reporter training. Educational documentation for the director and staff verified completion of the required ECE units.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:

LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. The center completed its water lead testing on June 14, 2022.

The Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. The Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed AB1207, the Child Abuse Mandated Training, with the Director. As of January 1, 2018, all staff must complete Mandated Reporter Training every two years. The training is available online at www.mandatedreporterca.com.

Pesticide regulations were discussed with the Director. In accordance with the Healthy Schools Act, California law requires that anyone using pesticides be trained annually in integrated pest management and the safe use of pesticides around children. The director was advised that a free one-hour online course is available on the Department of Pesticide Regulation's Web site: www.cdpr.ca.gov/schoolipm/training.
