On June 17th, 2025, at approximately at 9:45am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced compliant investigation inspection. LPA met with Site Supervisor Alicia Young and Assistant Director, Jeannine Solis and explained the purpose of the inspection. Present in the facility today were Site Supervisor and 16 staff supervising 94 preschool age children. During today’s inspection, LPA interviewed four staff and inspected the facility for Health and Safety Hazards.

During the course of the investigation, LPA conducted interviews and received pertinent documentation. Based on the interviews and documentation that was received, it has been found that the allegation of ‘lack of supervision resulting in day care child eloping’ has occurred.

On June 6th, 2025, LPA received an Unusual Incident Report documenting that on May 29th, 2025, Child#1 (C1) had climbed over the playground fence leaving daycare premises. Based on interviews conducted, staff observed C1 climbing the fence and verbally told child to get off. Staff stated that their efforts of verbally telling C1 to get off were not successful and C1 climbed over completely and ran towards the residential street. Staff stated that the child made it outside the gate to the driveway of the first house on the street next to the facility. Staff stated that the C1’s parents were notified.

Facility has put in a maintenance request to install privacy slats in the fence to prevent children from climbing the fence.

Facility will submit the supervision policy and how they plan to prevent these incidents from recurring.

Based on interviews and record review, a child (C1) was able to climb over the playground fence leaving the premise without the direct supervision of staff. This poses an immediate health, safety and personal rights risk to children in care.

Facility will have a training about Supervision policy and practices to ensure that all staff are aware of the supervision policy.

Facility will submit to LPA Tapia-Mandujano both the written plan and the training notes with signatures obtained of all staff present by July 18th, 2025.

Based on facility reporting incident and the interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

California Code of Regulations, Title 22, Division 12, are being cited. Please refer to 9099D for more information.

LPA Tapia-Mandujano informed facility representative, Alicia Young and Assistant Director, Jeannine Solis, that this report dated June 17, 2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Tapia-Mandujano informed the facility representative to provide a copy of this licensing report dated June 17, 2025 that documents any Type A citation to parents/guardians of all children currently enrolled including the Arundel Preschool children who are temporarily being housed by Brittan Acres, by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A plan of correction was also discussed. Facility was also notified that an office meeting may be scheduled at a later date.

Notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Assistant Director, Jeannine Solis. Appeal rights were provided during visit. This report is public and can be reviewed.
