On December 9th, 2025, at approximately 8:22am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced compliant investigation inspection. LPA met with Assistant Director, Jeannine Solis and Site Supervisor, Carina Neumann and explained the purpose of the inspection. Present in the facility today were Site Supervisor and 9 staff supervising 54 preschool age children. During today’s inspection, LPA inspected the facility for Health and Safety Hazards.

During the course of the investigation, LPA conducted interviews, observation, and received pertinent documentation. Based on the interviews, observations, and documentation received, it has been found that there is more than 51% evidence to prove that the allegation of staff are operating out of ratio has occurred.

Based on the interviews, observation, and documentation received, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

Facility must adjust schedule of staff/parents to ensure that no staff are left operating over staff to child ratio.Facility will submit an updated LIC 500, Personnel Report.

Facility must also submit proof of the schedule change of staff that has happened since the beginning of the year.

Based on observation and record review, the licensee did not comply with the section cited above. During the course of the investigation, it was determined by interviews, observations, and documentation that staff have been operating over staff to child ratio which poses an immediate health, safety or personal rights risk to persons in care.

Facility will implement appropriate procedures to ensure staff are not operating over staff to child ratio. Written procedures should include what steps staff should take when they are about to meet 1 to 12 ratio and when teachers are absent/running late.

A meeting must be conducted to ensure that all staff are made aware of the policies. Staff acknowledgement of meeting must be submitted to LPA after it is conducted.

Facility will submit written procedures and updated Personnel Report to LPA Tapia-Mandujano by 12/12/25.

LPA Tapia-Mandujano informed facility representatives, Jeannine Solis and Carina Neumann that this report dated December 9th, 2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Tapia-Mandujano informed the facility representatives, Jeannine Solis and Carina Neumann to provide a copy of this licensing report dated December 9th, 2025 that documents any Type A citation to parents/guardians of all children currently enrolled children by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A plan of correction was also discussed.

Notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Assistant Director, Jeannine Solis and Site Supervisor, Carina Neumann. Appeal rights were provided during visit. This report is public and can be reviewed.
