On March 24, 2026, Licensing Program Analyst (LPA) Van conducted an unannounced annual inspection and met with Director Raeli Aguirre. The purpose of the inspection was explained, and the director granted LPA access to the facility. This is a combination center with an Infant and Preschool program with a toddler component attached. Today’s inspection focused on the Preschool program with the toddler component.

At the time of the inspection, six teachers and the director were supervising 54 children. Of the 54 children present, 11 were enrolled in the toddler component, and 43 were preschool-age children. The center was operating within its licensed capacity and maintaining the required staff-to-child ratios.

All staff present during the inspection had their criminal records verified through the CDSS Guardian Background Check system and were found eligible.

The center operates Monday through Friday from 7:00 a.m. to 6:00 p.m.

The center uses the Building Kidz Connect electronic sign-in/sign-out system, with each family member assigned a unique access code. A review of attendance records confirmed that all children were properly accounted for and that all parents completed the required signing procedures.

LPA and the director conducted a walkthrough of the center to assess potential health and safety risks. The Preschool program operates in four classrooms: the Toddler Option (18 to 36 months), PS1 (2.5 to 3.5 years), PS2 (3.5 years to entry into first grade), as well as a library and a multi-purpose space. All classrooms were observed to be clean, well-organized, and maintained at a comfortable temperature with adequate ventilation. Age-appropriate toys and learning materials were accessible, and all furniture was observed to be sturdy and in good condition. The center is equipped with a combination smoke alarm system, multiple carbon monoxide detectors, and several fully charged 3A:40BC fire extinguishers.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed that each child has their own cubby for personal belongings. Sleeping cots were properly stored when not in use. According to the director, all bedding is provided by families and washed at the end of each week. Emergency supplies were stored in locked areas to ensure they were secure and inaccessible to children.

Medications were stored in high cabinets and on top of the refrigerator inside a plastic container with a snap-on lid. All medications were in their original containers and had the required parental consent forms. LPA reminded the director to periodically check all medications to ensure none have expired.

The Toddler component classroom has access to an exclusive restroom. Classrooms PS1 and PS2 share a restroom, which was observed to be clean and in proper working order. All bathrooms contained appropriate sanitation supplies and age-appropriate toileting equipment. Each restroom includes a diaper-changing station positioned within arm’s reach of an adult sink. A separate staff bathroom was also available on site. The director confirmed that no firearms were stored at the facility.

Disinfectants, cleaning solutions, poisons, and other hazardous materials were stored out of children’s reach. All accessible cabinets and drawers in the classrooms were checked and found free of hazardous items.

Sleep logs for children enrolled in the toddler option component were reviewed during the inspection. The 15-minute sleep checks were consistently documented, and the logs reflected the required check intervals, each child’s name, the corresponding dates, and staff initials.

The center has separate outdoor play areas for preschool children and those in the toddler option component. LPA observed that both outdoor areas were fully fenced and securely enclosed. There was an adequate supply of outdoor toys and equipment, all of which appeared to be in good condition. The outdoor play structures were equipped with cushioning material underneath. Additionally, all poles in the outdoor areas were padded for added safety. LPA did not observe any pools, spas, or other bodies of water on the premises.

According to the director, the center provides morning and afternoon snacks, and children bring their own lunch. All solid waste containers observed had tight-fitting lids. Children bring their own water bottles, and the center provides water for refills throughout the day.

All required postings, including the Facility License, Outdoor Waiver, Notification of Parent Rights, Personal Rights, Car Seat Law, Emergency Disaster Plan, and Daily Activities, were posted.

The director stated that the center conducts fire and earthquake drills at least once every six months; the most recent drills were conducted on February 10, 2026.

LPA reviewed a sample of children’s and staff records during the inspection. Children’s files were complete and included required documents such as immunization records, admission agreements, emergency contact information, and personal rights forms. Staff files showed that all employees and individuals subject to background checks had received the appropriate criminal record and child abuse index clearances or exemptions. Multiple staff members held current CPR certifications, maintained required immunization records, and were up to date on mandated reporter training. The director’s and staff’s educational documentation confirmed completion of the required ECE units.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.



CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:

LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

The Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. The Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with the director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed AB1207, the Child Abuse Mandated Training, with the Director. As of January 1, 2018, all staff must complete Mandated Reporter Training every two years. The training is available online at www.mandatedreporterca.com.

Pesticide regulations were discussed with the Director. In accordance with the Healthy Schools Act, California law requires that anyone using pesticides be trained annually in integrated pest management and the safe use of pesticides around children. The director was advised that a free one-hour online course is available on the Department of Pesticide Regulation's Web site: www.cdpr.ca.gov/schoolipm/training.
