25 | On March 21, 2025 at approximately 8:45AM, Licensing Program Analyst (LPA) Alvarado conducted an unannounced, case management visit to 2nd Generation Cabot (Preschool) in regards to an incident that was self-reported March 14, 2025. LPA met with Executive Director, Holly Anderson(D1), and explained the purpose of the visit. LPA Alvarado along with (D1) Conducted a Health and safety inspection for the Preschool license.
Under the preschool license, present during LPA's visit included 7 staff members supervising 34 children (Preschool Age). During LPA's visit, preschool children were transitioning during morning arrival.
On March 14, 2025, Teacher 1 (T1) and (T2) were present in the “Crocodiles” preschool classroom. C1 at approximately 12:23PM was leaving the restroom area located in the classroom when another child closed the door on C1. Teachers 1(T1) and 2 (T2) were transitioning from outdoors to indoors, and getting ready for lunch.
During today’s visit at approximately 10:21AM, LPA spoke with (T2) to gather more information regarding the self-reported Incident that occurred with C1. During today's visit, LPA observed that the classroom the bathroom area is a half wall, where supervision is maintained. The restroom door also does not lock, but can swing, and the door also has a finger safe protector. C1 has since returned to facility the day after the incident, as of March 14, 2025. C1 has returned with no restriction.

No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted for 30 days.
