On November 6, 2025, at approximately 10:30am, Licensing Program Analyst (LPAs) Maria Olguin-Leon and Leslit Tapia-Mandujano conducted an unannounced complaint investigation to deliver the findings of the above allegation. LPAs met with licensee, Edelmira Cardenas. Present during the inspection was licensee and assistant caring for 7 children (4 infants & 3 preschool age). Facility is operating within capacity of staff/child ratios. All adults present are fingerprint cleared and associated to the facility.

During the course of the investigation, LPA conducted interviews and reviewed pertinent information and conducted observations. Based on information gathered the allegation, Licensee restrained day care children, was determined to be SUBSTANTIATED.

A type “A” violation was issued today in accordance to the California Code of Regulations, Title 22, Division 12. Please refer to 9099D for more information.

LPAs Maria Olguin-Leon and Leslit Tapia-Mandujano informed Licensee, Edelmira Cardenas, that this report dated November 6, 2025, documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPAs Maria Olguin-Leon and Leslit Tapia-Mandujano informed Licensee, Edelmira Cardenas, to provide a copy of this licensing report dated November 6, 2025 that documents any Type A citation to parents/guardians of all children currently enrolled, by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with Licensee, Edelmira Cardenas. Appeal rights were provided. This report is public and can be reviewed.

Based on information gathered there is not sufficient evidence to prove the allegations: Licensee locked children in a room as a form of discipline, Licensee yells at day care children, Licensee placed infant in an unsafe environment, Licensee placed dangerous chemicals within reach of day care children and Licensee operated over capacity.

Although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED

Based on observation and record review gathered during complaint investigation, the licensee did not comply with section cited above, as LPA received evidence that licensee restrained day care children, which poses an immediate health, safety or personal rights risk to persons in care.
