13 | On January 8, 2025, at approximately 9:40 am, Licensing Program Analyst (LPA) Maria Olguin-Leon and Licensing Program Manager (LPM) Daniel Oquendo conducted an unannounced inspection and met with Director, Yasha Nasiripour. Purpose of the inspection was explained and was to deliver complaint findings for the above allegation. Present in the facility were Director, 3 staff caring for a total of 15 preschool children. Facility is operating within capacity. During today's inspection, LPA, LPM and Director inspected the facility for health and safety hazards.
During the course of the investigation, LPA inspected facility, conducted interviews, conducted observations, and reviewed pertinent documentation provided. Based on evidence obtained, it was determined that the allegation, Teacher shoved a child and child fell to ground, is determined to be UNSUBSTANTIATED. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Upon receipt of this report, Facility shall post the Notice of Site Visit for 30 consecutive days. Failure to maintain posting as required will result in an immediate $100 civil penalty. This report is public and can be reviewed.
Exit interview was conducted and report was reviewed with Director, Yasha Nasiripour and Appeal rights were provided.
