Licensing Program Analysts (LPAs) Glenn Schnell and Sheran Lo met with Applicants Nicole and Matthew Feldman today for a pre-licensing inspection due to a change of ownership. The applicants have applied as a Limited Liability Corporation. This inspection included a technical assistance inspection for COVID-19 guidance and support. This facility is located at Roosevelt Elementary School (Jefferson School District), currently a closed elementary school due to the COVID-19 pandemic. The applicant is requesting a combination center; 54 preschool children (ages 2 years to entry into first grade) and 60 school age children (ages 5 to 13 years old) . The facility will operate both programs Monday - Friday; 6:30 AM to 6:00 PM. The designated director/site supervisor for the facility is Nicole Feldman and her back up site supervisor will be Michelle Harris. The facility was inspected today, indoor and outdoor, for health and safety hazards and measured to calculate capacity.

Indoor: There are two rooms (700A and 700B) that will be used for the preschool program and the following square footage was calculated for each room: Room 700A 1,158.028 sq ft. , Room 700B 953.021 sq ft. Total useable indoor square footage is 2,111.049 sq ft divided by 35 sq ft equals 60 children. Room 700B will only be used for part time preschool care during the hours that school age children are in school. When room 700B cannot be used for the preschool children, the maximum capacity for the preschool must drop to a capacity of no more than 33 children. Room 700B will be used by part time preschoolers. The indoor space for children is observed today to be clean and all furnishing, toys and equipment observed to be safe and in good condition. Report continues on next page.

There is storage space for children's personal belongings and cots to be utilized for napping children. Applicant will provide a sheet and launder them daily. Parents will bring blanket for their child and it will sent home to be laundered daily during Covid. Cots will be cleaned by applicant at least weekly or more often if needed. The indoor space is equipped with a fire extinguisher and smoke detectors. In room 700A, there is a separate kitchen area with a dishwasher, sink with hot and cold water, and a refrigerator. Food is stored in the kitchen. Facility has garbage cans with tight-fitting lids, and a first aid kit. The facility is adequately ventilated and free of insects and bugs. Window screens are in good repair. The program provides two snacks and a lunch daily prepared by staff on site. Facility has a dishwasher for adequate sanitization of dishes and utensils. Disposable dishes and utensils will be used during Covid.

Foods will be stored in the kitchen separately from any cleaning products or other hazards. Medications will be stored in the kitchen high cabinet or refrigerator inaccessible to the children. There are 5 toilets ( 75 capacity) and 5 sinks ( 75 capacity) accessible for children's use; Three sinks and toilets are located in room 700A and two sinks and toilets are located in room 700B. Bathrooms are located inside the classrooms. The bathrooms were observed to be clean and with no health and safety hazards. Sinks accessible to children deliver water that is no warmer than 120 degrees. Hand soap and disposable paper towels are accessible to children. Facility has accessible drinking water, however, children bring their personal water bottles and are able to access as needed. All toxins and other hazardous items will be stored in a locked cabinet in the classroom and will be inaccessible to children. Isolation of ill children will be located in the director’s office and staff bathroom will be used for an ill child. Report continues on next page.

Outdoor: There is an enclosed outdoor space that will be used for the preschool children, and the applicants are also requesting to use the elementary school kindergarten program playground. The outdoor area for the preschool playground measures 2,462.74 sq ft.(32 children) and the Kindergarten playground measures 7,308.692 sq ft. (97 children) Total outdoor capacity is 9,771.432 (130 children). LPA Schnell has already received the original request for a waiver to share the kindergarten playground. Waiver will be granted for the preschool to use the kindergarten playground. Both outdoor areas to be used is enclosed by at least a four foot fence and observed to be clean, safe and equipped with toys and equipment for children. Resilient material located under climbing applicable climbing structures is wood chips. For outdoor drinking water, children will have their water bottles with them. Shade for children will be provided by over hang attached to the building.

The following items were reviewed as part of today's visit: Record keeping for staff, children and facility records, Care and Supervision of the Children, Child Discipline Procedures- staff will talk with the children and use redirection, Emergency Evacuation Procedures, Medication Policies, Isolation of Sick Children, Napping Requirements, Food Service, Transportation-none provided, Parents Rights, and Reporting Requirements. Children will be signed in and out by parents manually. Required postings were observed during today's inspection. Facility will conduct monthly disaster drills and a log will be maintained for review upon request. The fire inspection was conducted, and approval was received on 10/16/20. The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Information was provided to applicants during today's inspection. Advisory Note provided to applicants stating that the requirement for testing must be met by January 2023. Applicants have submitted a water test completed for the Elementary School. The rooms tested on the Lead Report indicates that room 22 and 23 are rooms 700A and 700B. Report continues on next page.

This facility will provide Incidental Medical Services – IMS. LPA reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. LPAs Schnell and Lo conducted a pre licensing inspection today and the inspection included providing the applicants with the COVID-19 self assessment guide. LPA also reviewed COVID-19 required postings and provided additional COVID-19 resources to applicants. LPA reviewed this guide with the applicants and requested the guide be completed and returned to LPA as part of completing the application process for licensure. The applicant has submitted a letter from the school district giving permission to operate on the premises during the pandemic while the school is closed. Applicant was informed that access to available Personal Protective Equipment (PPE) may be available through the local child care resource and referral agency.
