25 | On April 23rd, 2024 at 2pm, Licensing Program Manager (LPM), Marie Rodriguez, and Licensing Program Analyst (LPA), Leslit Tapia-Mandujano, met with director, Tajmah Martin for an informal office meeting. Present during meeting included Executive Director, Kira Lewis Clarke and Dean of Operations, Reyna Nieto.
The purpose of the meeting was to discuss the incidents that resulted in children being left unsupervised. Facility self reported incidents that occurred on October 4th, 2023, January 19th, 2024, and February 28th, 2024. On October 4th, 2023, a child was left outside on the outdoor play area for approximately two minutes as the rest of the class was transitioning back inside the classroom. On January 19th, 2024, a group of children went back into the classroom during outdoor play time with a staff, the staff and the other children went back outside closing the door behind them leaving one child inside for approximately one to two minutes. On February 28th, 2024, a child was left inside of the classroom for approximately three minutes as the children were transitioning to outside play area.
All three incidents occurred resulted in children being left without any direct supervision resulting in violating the supervision regulations. One type B and two type A deficiencies were assessed. Civil penalties were also assessed as it was a repeat violation in a span of over 4 months.
During today's visit, LPM discussed the citation and civil penalties issued for incident. The Plan of Correction are as follows:
- New Supervision and Transitioning Policies were developed and are being reinforced by Director
- Facility has held multiple trainings on Supervision Policy and Transitioning Techniques with staff
- Facility will continue to discuss Supervision and Transitioning Polices during all staff meetings
- Facility has submitted an Action Plan to ensure children are directly supervised at all times
- Facility has updated Policies to include Disciplinary Action for Staff that results in lack of supervision
Deficiency issued during case management inspection visit has been cleared with today’s office meeting.
