On February 21st, 2025, at approximately 9:15am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced random/annual and Case Management inspection and met with licensee, Yongli Chen. LPA explained the purpose of the inspection licensee. Present in the facility are licensee and assistant caring and supervising 5 children (1 infant, 3 preschoolers, and 1 school- age child). During today's inspection, all adults living and/or working in the facility are fingerprint cleared and associated.

Licensee has a pending Increase of Capacity application. Increase of Capacity application was submitted on October 4th, 2024 to the department. Fire Clearance was approved by Menlo Park Fire Department on February 4th, 2025.

Licensee owns three-bedroom three-bathroom house with an Accessory Dwelling Unit (ADU) in the back (999A). Licensee lives in the home with husband and adult daughter. The hours of operation are seven days a week, 24 hours a day. Daycare areas are: Living Room, Dining Room, Bedroom #3 (currently not being used), Bathroom #3, and portion of the fenced backyard. Applicant will occasionally take the children to the front yard (to the left) with the gates closed and coned off areas. OFF limit areas are: Bedrooms #1 & #2, Bathroom #1 & #2, Office, Kitchen, Laundry Room, Garage, and the ADU (999A). All off-limit areas are properly barricaded, including closets.

LPA inspected home for health and safety hazards. LPA observed the Day-care is clean, orderly with a variety of age-appropriate toys for the children. All furniture inspected is in good repair. The applicant has a fully stocked First Aid kit and was reminded to be aware of the expiration on the First Aid Kit. The home has no pools or bodies of water in the home. LPA reminded licensee about water safety. The home has a fireplace that is properly barricaded. Per licensee there are no pets in the home. The home has age-appropriate equipment available for children in care.

Licensee was reminded that baby walkers, bouncers, jumpers, and any other similar items are to not be used for children in care. Per licensee, she sleeps children on cots in the living room. Licensee currently does not have children under 12 months but has age-appropriate sleeping equipment if needed. Sleep logs were not maintained. Technical Violation was issued.

There was a fully charged fire extinguisher and a smoke and carbon monoxide alarm, and a working telephone. Phone number listed for Licensee is current. Per Licensee, there are no weapons or firearms in the home. LPA reviewed five children’s records which were complete. Licensee's Mandated reporter training will expire on 10/2025. Licensee took Pediatric CPR/First Aid training that is not EMSA certified. Technical violation was issued. Facility just started to enroll children; an emergency drill has not been conducted. Technical violation was issued. LPA reminded licensee, emergency drills must be conducted at least once every six months and must be properly logged.

All the required posting documentation, such as the facility license, Notification of Parental Rights and have been placed in a prominent area for parents or representatives to review. Discipline Policy was also discussed.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. Licensee does offer IMS at this moment. Incidental Medical Services plan is on file. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee, Yongli Chen, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS on 2/21/25.

LPA will approve a Large Family Child Care License as of today, February 21st, 2025.

No Deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with License, Yongli Chen and copy of this report was provided.
