On January 13, 2026 at approximately 9:50am, Licensing Program Analysts (LPAs) Quimbo and Olguin-Leon conducted an unannounced, complaint visit. LPAs met with director, Jessica Smith, and explained the purpose of the visit.

Facility is a multi-component facility licensed for an infant, toddler and preschool programs. Present during LPAs’ visit included 50 children and 11 teaching staff (including director and 1 substitute).

LPAs observed 8 infant children with 2 teaching staff (in Caterpillars room), 11 toddler children with 3 teaching staff (in Ladybugs room) and 31 preschool children with 5 teaching staff (15 children with 3 staff in Honeybees room and 16 children with 2 staff in Dragonflies room).

During today’s visit, LPAs interviewed 8 teaching staff, and obtained and reviewed facility records. LPAs requested to review camera footage of self reported incident. Director stated they did not have access to camera footage. At approximately 1:30pm, licensee, Navin Chugh, arrived to the facility.

Facility self reported an incident that occurred on January 9, 2026. Teaching staff was observed to grab an enrolled child, pick up the child and force the child to sit down in the classroom. Per staff interviewed, child did not obtain injuries and child’s parent was informed of incident. Selection of staff interviewed also stated they have observed a staff member inappropriately handle a child in care. Per Family Handbook, children are disciplined through redirection.

Licensee self reported incident to the department January 12, 2026. An emergency staff meeting was conducted January 12, 2026. Proof of meeting was provided to LPAs during today’s visit. Staff member who inappropriately handled a child received a write up from director and was also terminated. Per staff interviewed, child continues to attend facility as per their normal.

Based on record reviews and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

Facility is being cited a Type A citation for a staff member inappropriately handling a child in care. A civil penalty was assessed due to repeat violation within 12 months.

LPAs Quimbo and Olguin-Leon informed licensee, Navin Chugh, and facility director, Jessica Smith, that this report dated January 13, 2026 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPAs Quimbo and Olguin-Leon informed the licensee and facility director to provide a copy of this licensing report dated January 13, 2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Appeal rights were also provided during visit. A plan of correction was discussed.

Facility self reported unusual incident to the department. Staff member who inappropriately handled a child received a write up from director and was terminated from facility.

Based on interviews and record reviews, a staff member was observed to grab a child, pick the child up and force the child to sit down in a rough, inappropriate manner. This poses an immediate health, safety or personal rights risk to children in care.

Civil penalty was assessed. Staff will have an additional training through the department on how to appropriately handle children. LPAs will refer director to child advocacy for personal rights training.
