Facility self reported an incident that occurred on January 9, 2026. Teaching staff was observed to grab an enrolled child, pick up the child and force the child to sit down in the classroom. Per staff interviewed, child did not obtain injuries and child’s parent was informed of incident. Selection of staff interviewed also stated they have observed a staff member inappropriately handle a child in care. Per Family Handbook, children are disciplined through redirection.
Licensee self reported incident to the department January 12, 2026. An emergency staff meeting was conducted January 12, 2026. Proof of meeting was provided to LPAs during today’s visit. Staff member who inappropriately handled a child received a write up from director and was also terminated. Per staff interviewed, child continues to attend facility as per their normal.
Based on record reviews and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
Facility is being cited a Type A citation for a staff member inappropriately handling a child in care. A civil penalty was assessed due to repeat violation within 12 months.
LPAs Quimbo and Olguin-Leon informed licensee, Navin Chugh, and facility director, Jessica Smith, that this report dated January 13, 2026 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPAs Quimbo and Olguin-Leon informed the licensee and facility director to provide a copy of this licensing report dated January 13, 2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Appeal rights were also provided during visit. A plan of correction was discussed.
